  1. PhysioFit Health strongly recommends that you consult with your physician before beginning any exercise program. You should be in good physical condition and be able to participate in the exercise.
  2. You should understand that when participating in any exercise or exercise program, there is the possibility of physical injury. If you engage in this exercise or exercise program, you agree that you do so at your own risk, are voluntarily participating in these activities, assume all risk of injury to yourself, and agree to release and discharge PhysioFit Health from any and all claims or causes of action, known or unknown, arising out of PhysioFit Health

You agree to the following:

That I am participating in physical activity at PhysioFit Health & Sports Injury Clinic which may include but is not limited to Pilates. I recognize that any physical activity may be strenuous and may cause injury, and I am fully aware of the risks and hazards involved in such activity. I understand that the use of exercise equipment also carries with it a risk of injury. I recognize that many changes occur as a result of exercise lessons, including possible short-term aggravation of some symptoms, feelings of tiredness, lightheadedness, increased energy, mood changes, etc.

I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in these classes. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in these classes. If I have any existing medical condition, I have been cleared by my doctor to participate in activities at PhysioFit Health and explained the details on the New Patient Enrollment Form and in person. If I have an existing medical condition that has been cleared by a physician, that I have and will continue to keep PhysioFit Health fully informed of any physical condition or disability, which would prevent or limit my participation in an exercise or physical conditioning program.

In consideration of being permitted to participate in these classes, I agree to accept full responsibility for any risks, injuries or damage, known or unknown, which I might incur as a result of participation in these activities or as a result of negligence. In further consideration of being permitted to participate in these classes, I knowingly, voluntarily and expressly waive any claim I may have against PhysioFit Health for injury or damages that I may sustain as a result of participating in these activities. 

PhysioFit Health shall not be responsible or liable for any articles lost, stolen or damaged, in or about the studio. I understand that all classes require prior evaluation by a physiotherapist.

I have read the above release form and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above. 

CANCELLATION POLICY: PhysioFit Health respectfully requests a 3-hour notice of cancellation. I understand that if I must cancel a scheduled appointment, I must call as soon as possible, and if I can’t cancel with enough notice that I will be held responsible for payment in full or forfeit a prepaid class.

All prepaid services (including, but not limited to, multiple session packages) are non-refundable and non-transferable, and must be used within the allotted time; any unused portions will be forfeited.
